Man on Fire Spent shell does not discharge
In the Movie Man on Fire, Denzel Washington puts a gun to his head and pulls the trigger. The bullet end is dented like a spent shell but it does not go off, or else the movie would be over .
Denzel is in awe of this event perhaps believing that God intervened?
How probable is this to happen in reality?

When you purchase cheapie Remington .22 LR ammunition in bulk 500 lb. packages you probably will run across about a half dozen to dozen "dudds" in that package. You will get a hit on the rim and indentation but it won't fire.
My Colt Phython .357 Mag. revolver is prone to misfire if the hammer area is not cleaned inside periodically. I would say about every 500 rounds it needs a real going over. It either needs to be dismantled and cleaned or dipped at the gunshop. Powder residue and cleaning oils can build up and cause trouble. When this happens the revolver may misfire about 1 shell out of 6 and does leave a firing pin hit on the primer.
This can also happen on semi-auto pistols if they are not cleaned correctly. There is more to cleaning a weapon then running a swatch down the barrel and wiping the outside down.
I'm don't always go the extra in cleaning the inner workings like I should. When in life threatening situations like the military or police work, it is best to take it totally apart for routine cleaning. Usually you can feel the regression in performance if you fire it frequently. Desert and jungle environments may make this necessary every day or even several times a day.
I didn't see the movie, but one of the CSI Miami shows had an episode where one of their characters died off from being in a shoot-out and his weapon misfired from lacking of cleaning.
Centerfire ammo is generally more reliable than rimfire.
